RN Stroke Program Coordinator

Coordinates facility-wide stroke program accreditation and credentialing in accordance with state and national standards. Supports a nationally recognized stroke program with AHA Gold Plus and Silver Plus designations and an expanding regional Telestroke network.

Serves as a clinical leader for stroke care, developing and implementing evidence-based pathways, monitoring patient outcomes, identifying care variances, and driving quality improvement initiatives. The incumbent is responsible for educating staff who care for stroke patients, working collaboratively with WJMC and LCMC leadership to ensure high-quality patient care and adherence to stroke program standards.

EXPERIENCE QUALIFICATIONS

• 3 years of experience Clinical nursing

• Required: Associate’s degree in nursing

Stroke Coordinator Responsibilities

40% | Stroke Program Coordination & Patient Care

• Lead hospital-wide stroke activations, providing rapid assessment, triage, and treatment coordination.
• Collaborate with the interdisciplinary team to ensure quality stroke care, regulatory compliance, patient progression, and discharge planning.
• Participate in stroke rounds, committee activities, data reporting, and performance improvement initiatives.
• Coordinate community outreach, staff education, and stroke awareness programs.
• Support stroke data collection, analysis, and program optimization with Stroke Program leadership.

20% | Clinical Practice & Professional Collaboration

• Serve as a resource for current stroke guidelines and evidence-based practice.
• Provide education, training, and compliance monitoring for staff involved in stroke care.

10% | Patient & Family Education

• Develop, maintain, and evaluate patient education materials.
• Educate patients and families on stroke prevention, treatment, recovery, and discharge planning.
• Assess learning needs and provide individualized education.

15% | Leadership & Program Development

• Act as a clinical leader and consultant for stroke patient management.
• Participate in developing and maintaining stroke standards, protocols, and procedures.
• Collaborate with leadership and interdisciplinary teams to improve patient outcomes and care coordination.

10% | Communication & Professional Relationships

• Demonstrate effective communication and collaboration with patients, families, physicians, and healthcare team members.
• Maintain confidentiality, professionalism, and a patient-centered approach.
• Respond constructively to feedback and foster positive working relationships.

5% | Professional Development

• Maintain active involvement in professional stroke or neuroscience organizations.
• Participate in continuing education, conferences, and review of current research to support best practices in stroke care.
