Intermountain is expanding and we’re hiring! We are looking for an energetic, adaptable, purpose-driven, and dedicated person to join our team as our Outpatient Case Manager. To be successful in this role, you will:
- Serve as the primary liaison between families, guardians, community partners, and the treatment team, ensuring clear communication and coordination throughout treatment
- Coordinate referrals, assessments, admissions, treatment planning, case conferences, and discharge planning.
- Collaborate with families, providers, and community agencies to support treatment goals and address barriers to progress.
- Provide families with education, resources, and support to strengthen engagement and improve outcomes for children.
- Maintain accurate, timely documentation and ensure compliance with agency, Medicaid, and regulatory requirements.
- Monitor treatment progress, facilitate ongoing review of care plans, and support continuity of services throughout the child's treatment journey.
Qualifications:
Required
- Graduation from an accredited college or university with a bachelor’s degree in social science or closely related field or equivalent experience.
- 19 years or older
- Valid Driver’s License
- Must pass a Background check prior to start date
