Short Description
The Sports Medicine Nurse Navigator serves as a key clinical resource and patient advocate, guiding patients throughout their orthopaedic sports medicine surgical journey. This role coordinates pre-operative and post-operative care, facilitates communication between patients and the care team, provides education and support, assists with discharge planning, and ensures timely follow-up to promote optimal outcomes. The Nurse Navigator works collaboratively with surgeons, advanced practice providers, rehabilitation services, and other healthcare professionals to deliver seamless, patient-centered care and enhance the overall patient experience.
Experience Required
Nursing experience preferred and program development background a plus.
