Part-Time and Per Diem (Every other Weekend) | $32.00 per hour

We are seeking compassionate and dependable Licensed Practical Nurses (LPNs) to join our Wellness team on a part-time or per diem basis. The LPN will provide hands-on resident care, administer medications, maintain accurate documentation, and collaborate with the Registered Nurse and care team to support residents’ health, safety, and well-being.
Responsibilities:
- Provide resident care in accordance with individualized service plans.
- Assist residents with activities of daily living as needed.
- Administer prescribed medications and accurately document all services provided.
- Monitor residents for changes in health, behavior, or functional status.
- Promptly communicate resident concerns or changes in condition to the Registered Nurse and appropriate healthcare providers.
- Assist with developing and updating resident service plans and clinical documentation.
- Respond to resident emergencies, provide first aid, and arrange appropriate medical follow-up.
- Maintain accurate records in communication logs, service plans, and daily care documentation.
- Provide compassionate one-on-one support to residents as directed.
- Help maintain a safe, comfortable, and respectful environment.
- Support the Registered Nurse with staff training, assignments, and other clinical responsibilities as needed.
- Follow all community policies, procedures, and applicable healthcare regulations.
- Promptly report resident incidents and work-related injuries.
Qualifications
- Current and active Licensed Practical Nurse (LPN) license in the State of New Jersey.
- Successful completion of an approved practical nursing program and required licensing examination.
- Ability to work collaboratively in a fast-paced healthcare environment.
- Strong communication, organizational, and documentation skills.
- Compassionate bedside manner and excellent attention to detail.
- Experience in assisted living, senior living, or long-term care is preferred.
Shift:
- 3:00pm-11:00pm
- 11:00pm-7:00am
- Every other weekend
