Hospice Nurse Practitioner
Hospice Nurse Practitioner Position:
Are you a compassionate, skilled clinician who feels called to make a difference in the lives of patients and families facing end-of-life care? Good Samaritan Hospice, an entity of Concordia Lutheran Ministries, is seeking a Hospice Nurse Practitioner to join our dedicated team.
As a valued member of our hospice care team, you’ll provide expert clinical support, symptom management, and compassionate guidance to patients and their loved ones. You’ll collaborate with physicians, nurses, social workers, and chaplains to ensure comfort, dignity, and quality of life are always at the heart of what we do.
What You’ll Do as a Hospice Nurse Practitioner:
Provide hospice patient assessments, symptom management, and clinical direction in collaboration with the hospice medical director.
Work closely with the interdisciplinary team to create and adjust individualized plans of care.
Offer education and emotional support to patients and their families.
Participate in team meetings and ongoing quality care initiatives.
Who you are as a Hospice Nurse Practitioner:
Current Nurse Practitioner license (CRNP) in the state of Pennsylvania.
Hospice and/or palliative care experience preferred.
A compassionate, patient-centered approach to care.
Excellent communication and collaboration skills.
