• Conduct Assessments & Develop Care Plans: Complete in-home assessments and create individualized plans of care to ensure clients receive the support they need.
• Support & Supervise Caregivers: Provide guidance, oversight, and ongoing support to Home Health Aides and Certified Nursing Assistants to ensure high-quality care and client satisfaction.
• Onboard & Educate: Facilitate new hire orientation and provide ongoing training, competency evaluations, and in-services to support caregiver success.
• Coordinate Client Care: Act as the central point of communication between clients, families, caregivers, and referral sources, ensuring continuity and quality of care.
• Promote Quality & Compliance: Help ensure care delivery meets BAYADA standards, client expectations, and applicable regulatory requirements.

What We're Looking For:

• Registered Nurse (RN) with a minimum of 2 years of nursing experience
• Previous home care experience preferred
• Experience supervising, mentoring, precepting, or supporting caregivers, nursing assistants, or other direct care staff preferred
• Strong communication, relationship-building, and organizational skills
